I fixed several frame buffer reference leaks in libaom and improved the
code that acquires and releases frame buffer references. I use
bug aomedia:2179 as the tracking bug for that work. Although I may have
neglected to associate a few CLs with that tracking bug, it should point
you to most of the CLs.

I should point out that most of the reference leaks were in error code
paths.

I also found extraneous references that are released correctly. They are
harmless but make the code quite confusing. I haven't dealt with that yet.
